Tramadol Adrenal failure: case report

A 65-year-old woman developed adrenal failure following treatment with tramadol. The woman had a history of arterial hypertension (HBP), dyslipidaemia, varices in lower limbs, depression, fibromyalgia, rheumatoid arthritis and herniated cervical, dorsal and lumbar discs. She had been on treatment with tramadol 200mg for 3 years [route and indication not stated] along with various concomitant medications. She had been assessed in the emergency department several times due to episodes of weakness and general malaise accompanied by arterial hypotension. Her antihypertensive therapy was discontinued. She was referred to the HBP unit. Lab tests showed decreased levels of ACTH and cortisol. Therefore, a diagnosis of adrenal failure related to tramadol was made [duration of treatment to reaction onset not stated]. The woman’s treatment with tramadol was withdrawn. At 3 months follow-up, ACTH and cortisol levels normalised. Gonzalvo Gomez I, et al. Adrenal insufficiency in a patient treated with tramadol. Atencion Primaria 51: 512-513, No. 8, Oct 2019.
