Atypical subtrochanteric fracture in an elderly patient: case report A 91-year-old woman sustained an atypical subtrochanteric fracture while receiving alendronic acid [route, dosage and therapeutic indication not stated]. The woman, who had been receiving alendronic acid for 3 years, was admitted in February 2009 after sustaining a low-energy subtrochanteric fracture with an unusual fracture pattern. She had previously presented in December 2006 at the age of 88 years with left hip pain, and plain radiographs had been deemed normal at the time. However, a review of her radiograph showed cortical hypertrophy in the subtrochanteric section corresponding to the site of the fracture. An antegrade nail was used to stabilise the woman’s fracture. Callus formation was observed to be slow 2 months later. There was still no evidence of fracture union in November 2009, and the distal locking screws were removed. Fracture union was noted 2 months later. Thompson RN, et al. Atypical femoral fractures and bisphosphonate treatment: Experience in two large United Kingdom teaching hospitals.
