Hypertensive crisis: 2 case reports Two women, who were treated with aripiprazole [routes and initial dosages not stated], developed hypertensive crises. A 51-year-old woman, whose past medical history included hypertension and bipolar-type schizoaffective disorder, was admitted to a hospital. During her admission, she was started on aripiprazole. Within 48 hours of starting aripiprazole, she developed confusion, headache and a maximum blood pressure of 198/100. In that time, aripiprazole had been titrated aggressively to 10mg twice daily. She was transferred to the cardiac ICU and managed successfully with labetalol. Aripiprazole was withdrawn, and her blood pressure stayed normalised. A 62-year-old woman, whose past medical history included hypertension, psychotic disorder and major depressive disorder, was admitted to a hospital. During her admission she was started on aripiprazole. Throughout her admission, her blood pressure was elevated. Within 72 hours of starting aripiprazole, her blood pressure increased to 200-210/90-110. Her blood pressure peaked following an aripiprazole dose increase to 10mg daily. Her aripiprazole dose was decreased, but her blood pressure remained elevated. Subsequently, her aripiprazole was withdrawn [outcome not stated]. Author comment: "[A] temporal relationship was observed between initiation of aripiprazole and hypertensive crisis." Sloan Hepfer K, et al. Aripiprazole-induced hypertensive crisis: A case series.
