Acute dystonic reaction: case report A 16-year-old boy experienced an acute dystonic reaction during treatment with aripiprazole for acute mania. The boy, whose medical history was notable for bipolar-I disorder, was hospitalised for psychiatric evaluation after he jumped off a bridge as a dare and exhibited erratic and heedless behaviour concerning his own safety. Following admission, he exhibited acute mania; hence, he received oral aripiprazole 10mg at 0900 hours in the morning. However, at 1345 hours on the same day, he developed severe neck pain with inability to move his neck. Physical examination showed severe contraction of the right side of his neck, with torticollis on the left. He was tearful and visibly uncomfortable. An acute dystonic reaction to aripiprazole was diagnosed. The boy was administered benztropine; however, he continued to experience the severe pain and was unable to move his neck. Therefore, he received another dose of benztropine, after which his symptoms improved. He was able to move his head and neck, and appeared to be much more comfortable. Aripiprazole was immediately discontinued, and he started receiving quetiapine. He was eventually discharged on quetiapine and benztropine. Abdullayeva N, et al.
