Worsening psychotic symptoms?: case report A 29-year-old man developed worsening of his psychotic symptoms during treatment with aripiprazole. The man had arrived at an inpatient unit after presenting with poor self-care, bizarre behaviour and sleep disturbances. After an episode of impulsive aggression, he began receiving aripiprazole which was titrated to 30mg/day for 1 week [route not stated]. His clinical status deteriorated [time to reaction onset not stated] and he developed disorganised thinking with loose associations and ideas of reference. He also became menacing toward other patients. The man received 4 intramuscular haloperidol injections over the following week; haloperidol was increased to 30 mg/day over the subsequent 10 days. Aripirazole was then withdrawn. He improved, but remained grossly psychotic. Author comment: "As in Mr. N’s case, it is not always possible to determine whether worsening psychosis is the natural disease course or a treatment effect. . . Because akathisia may be confused for worsening psychosis, it is possible akathisia was mistakenly identified as worsening psychotic symptoms in Mr. N’s case". Gugger JJ, et al.
