First report of Marchiafava-Bignami disease: case report A 51-year-old non-alcoholic woman with basoloid carcinoma of the anus developed Marchiafava-Bignami disease (MBD) following treatment with cisplatin [route, dosage and duration of treatment to reaction onset not stated]. The woman presented with confusion, agitation, visual hallucinations, apraxia and aphasia after receiving cisplatin and fluorouracil. An MRI of the brain revealed MBD, characterised by a new hypertense signal in the splenium of the corpus callosum. Other aetiologies causing similar MRI findings were excluded. Cisplatin and fluorouracil were discontinued. The woman received vitamin supplementation, and returned to baseline after a few weeks. A repeat MRI 9 weeks later revealed complete resolution of the abnormality. Editorial comment: A search of AdisBase, Medline, Embase and the WHO ADR database revealed no previous reports of Marchiafava-Bignami disease associated with cisplatin.
