Acute kidney injury secondary to crystal nephropathy: case report A 37-year-old man developed acute kidney injury due to crystal nephropathy during treatment with levofloxacin. The man was hospitalised with oligo-anuria. He had received IV levofloxacin [indication not stated] 500mg once a day for 5 days with cumulative dose of 2.5g. At the current admission, laboratory investigations showed WBC 10400 /mm3, haemoglobin 10mg%, serum creatinine 6.1 mg%, platelet count 1.8 lakhs/mm3, phosphorus 4.1 and calcium 9.1 mg%. Urine examination revealed pH of 6.1 with trace proteins, 24 hour urine proteins was 100 mg/day. His serum creatinine level increased to 7.5 mg%. Hence, renal biopsy was performed; and light microscopy revealed polygonal to needle shaped luminal, circumferential, refractory crystals in tubules, which revealed reddish white polarisation consistent with acute kidney injury secondary to crystal nephropathy induced by levofloxacin [duration of treatment to reactions onsets not stated]. The man received unspecified supportive treatment and adequate hydration. Ten days later, his condition improved.
