Levofloxacin Achilles tendinitis: case report A 20-year-old man developed Achilles tendinitis during treatment with levofloxacin for community-acquired pneumonia. The man initially received IV cefminox. No improvement was observed after 2.5 days and he was changed to IV levofloxacin 750mg once daily. Three days after the initial levofloxacin dose he experienced acute pain at the back of his left heel, 2-3cm proximal to the insertion of the Achilles tendon. The skin covering the tendon was swollen and red. The patient had difficulty moving his ankle, which was tender on light palpation. No other joints appeared affected. Levofloxacin-induced Achilles tendinitis was suspected. Levofloxacin was discontinued and replaced with IV ceftriaxone. Symptoms improved within a few days of the treatment change.
