Levofloxacin Arthralgia: case report A 36-year-old woman developed arthralgia while receiving levofloxacin for Helicobacter pylori infection. The woman was diagnosed with acid peptic disease five years previously. A biopsy confirmed diffuse chronic inflammation with H. pylori infestation. She had received treatment with different proton pump inhibitors such as omeprazole, lansoprazole and esomeprazole as available, but with no success. The symptoms persisted, and at the age of 36 years, she started receiving rescue treatment with levofloxacin 1g every 12h [route not stated] along with amoxicillin and lansoprazole for fourteen days. On treatment day 5, she reported generalised myalgia. On the subsequent day, she presented with arthralgia with limited movements of elbows and knees. An orthopaedist was consulted, who advised joint sparing treatment, physiotherapy for epicondylitis and isometrics for the quadriceps. Laboratory tests revealed vitamin D deficiency and presence of antinuclear antibody. On treatment day 7, the woman’s treatment with levofloxacin was stopped and she showed progressive improvement. The treatment advised by the orthopaedist was not administered considering progressive improvement. On Naranjo algorithm, score of 7 indicated probable relationship of arthralgia with levofloxacin.
