Tactile hallucination and acute anxiety: case report. A 19-year-old man developed tactile hallucination and acute anxiety during treatment with levoflaxacin for acute sinusitis. The man presented to the otorhinolaryngology department with a history of sneezing, postnasal drip, purulent nasal discharge, pain below both the eyes and fever since 5 days. On evaluation, he was febrile, had a bilateral maxillary tenderness, while other systems were apparently normal. The X-ray paranasal sinuses showed bilateral maxillary sinus haziness. He was diagnosed with acute sinusitis. He was started on treatment with levofloxacin 500mg tablet daily for 5 days [route not stated], paracetamol 500mg every 6 hours and nasal decongestant naphazoline drops daily four times. Within half an hour of the first dose of levofloxacin, he developed blurred vision for a few minutes and a feeling of insects crawling over his chest, face and upper limbs with itching. He was also confused and anxious. The symptoms continued for an hour and then subsided itself. Similar symptoms were experienced after the second dose of levofloxacin on the next day. However, after intake of paracetamol and naphazoline he had no symptoms. Never before he had experienced similar complaints. Hence, a provisional diagnosis levofloxacin induced tactile hallucination with acute anxiety was made. The man was withdrawn from the levofloxacin therapy and the episode of tactile hallucination and acute anxiety subsided on its own without any intervention. He did not experience any similar symptoms following discontinuation. He was switched to amoxicillin and clavulanic acid and was continued with the paracetamol and naphazoline therapy with no side effects. Maharani B, et al.
