Moebius syndrome and complex cardiopathy following in utero exposure: case report A 5-year-old girl presented with Moebius syndrome and complex cardiopathy after prenatal exposure to misoprostol. The girl presented for psychomotor and language developmental delays associated with congenital defects. Her mother [age not stated] reported receiving oral misoprostol 400mg and vaginal misoprostol 200mg during pregnancy at 6 weeks’ gestation [indication not stated]. The girl’s birth weight and height had been 2150g and 49cm, respectively. Upon current examination, she had the following conditions: bilateral epicanthal fold, bilateral facial paralysis of central origin, bilateral microtia, right hemifacial microsomy, macrostomy with right deviation of the oral commissure, flat philtrum, dental malocclusion, cutaneous syndactyly of the left hand with fusion of thumb and index finger as well as middle and ring fingers, and syndactyly of the first and second digits of both feet and third and fourth digits of the left foot. Her head circumference, weight and height were all below the fifth percentile. Echocardiogram revealed a ventricular septal defect and aortic pseudocoarctation. She was diagnosed with Moebius syndrome [outcome not stated]. Author comment: It can be concluded from this case that the anomalies which generated Moebius syndrome and complex cardiopathy may suggest a causal relationship to prenatal exposure to misoprostol through vascular disruption.
