Spontaneous breast haematoma: case report An 80-year-old woman developed spontaneous breast haematoma during treatment with warfarin for deep vein thrombosis (DVT). The woman, who had history of hypercholesterolaemia, had been receiving warfarin 3.75 mg/day [route not stated] for DVT of lower extremity from past 3 months. However, she was admitted with the sudden onset of bilateral breast swelling and pain, which progressively increased in size. She was afebrile with a heart rate of 90/min, a respiratory rate of 20/min and BP of 110/70 mmHg. Physical examination showed the presence of bilateral breasts swelling and ecchymosis along with the right arm, and the right flank. Her ecchymosis patches were tender, with no palpable mass. Subsequently, she was diagnosed with spontaneous bilateral breast haematoma. The haematoma had limited her activity and she was uncomfortable. Lab tests showed INR of 3 and haemoglobin of 10 g/dL. Warfarin was discontinued and heparin was initiated. The woman was treated conservatively with success. Her haematoma regressed progressively, and she was discharged after 20 days of hospitalisation. She was well at 6 months of follow-up. Shakerian B, et al. Warfarin-induced spontaneous bilateral breast and extrathoracic hematoma in an elderly woman.
