Fluid retention: case report A 67-year-old man experienced fluid retention during treatment with docetaxel for gastric cancer. The man with stage-IIIB gastric cancer (T3N3aM0), underwent curative distal gastrectomy. Postoperatively, he started receiving adjuvant chemotherapy as per JACCRO GC-07 regimen, which included gimeracil/oteracil/tegafur (S-1) administered on days 1–14 of a 3 week cycle during the first course. After the first course, he received IV docetaxel 40 mg/m2 body surface area on day 1 and gimeracil/oteracil/tegafur on days 1–14 of each 3 week cycle. He also received pretreatment with dexamethasone 15 min prior to the docetaxel administration, and prophylactic dexamethasone doses after docetaxel administration. After the fifth course, he presented to hospital with peripheral oedema and abdominal distension. Physical examination showed abdominal distension with shifting dullness. Laboratory findings were as follows: haemoglobin 8.8 g/dL, albumin 2.9 g/dL, carcinoembryonic antigen 4.2 ng/mL and CA19-9 11.3 U/mL. CT showed massive ascites. Abdominal paracentesis revealed that the ascites had a clear appearance. Laboratory findings of the ascites were as follows: lactate dehydrogenase 126 U/L, protein 2.6 g/dL, albumin 1.4 g/dL, glucose 172 mg/dL and serum-ascites albumin gradient (SAAG), 1.5. No microorganisms were detected in the ascites and cytology for the ascites was also negative. Based on the examinations, he was diagnosed with docetaxel-induced fluid retention. The man’s adjuvant chemotherapy was stopped. He received unspecified diuretics. One month later, the ascites and peripheral oedema improved. His serum albumin level was elevated. Thereafter, adjuvant chemotherapy was started with gimeracil/oteracil/ tegafur alone. He survived for 24 months following the surgery without any recurrence [immediate cause of death not stated].
