Dexamethasone Lack of efficacy: case report

In a case series of 3 patients, a 66-year-old woman was described, who exhibited lack of efficacy during treatment with dexamethasone for immune thrombocytopenia. The woman, who had hypertension, presented to the hospital with spontaneous epistaxis, petechiae and high blood loss from haemorrhoids since 3 weeks. She experienced dyspnoea, fever and coughing followed by diarrhoea and vomiting 4 weeks before the presentation. During the admission, she underwent PCR that showed COVID-19 infection and her platelet count was 2 x 109/L. Hence, she received one unit of platelets; but, her platelet count had not increased. She was diagnosed with immune thrombocytopenia related to COVID-19. Therefore, she started receiving dexamethasone [route not stated] 40 mg/day for 4 days. However, no response was obtained. Hence, the woman was treated with immune-globulin. Eventually, her platelet count increased to 32 x 109/L on hospitalisation day 22 Bomhof G, et al. COVID-19-associated immune thrombocytopenia.
