Dexamethasone Ocular hypertension following phakic intraocular lens implantation: case report A 24-year-old woman developed elevated intraocular pressure (IOP) after receiving dexamethasone postoperatively following phakic intraocular lens implantation in the left eye. The woman was treated with local 0.1% dexamethasone [dosage not stated] and ofloxacin four times daily after surgery. One week later, local treatment was reduced to dexamethasone twice daily. One month after surgery she presented with a sudden loss of visual acuity in the left eye. Her uncorrected visual acuity had decreased to 20/32 (from 20/25 1 week after surgery) with an astigmatic change of about 3 D of cyl from previous assessment. Her IOP was 45mm Hg (compared with 18mm Hg 1 week after surgery). Dexamethasone was discontinued, and the woman was treated with apraclonidine and oral acetazolamide. Three days later, she had an uncorrected visual acuity of 20/20 and IOP of 10mm Hg. Acetazolamide was discontinued but she continued to received apraclonidine for another month.
